Evolve Therapy Rooms Ltd is registered with ICO (Information Commissioners Office) which means we need to tell you what data we collect from you and what we intend to do with it.

What data do we keep and why do we keep it?

Name, date of birth and ethnicity:- This is basic information that helps us get to know you.

​

Address, email address, phone number:- We use this as a way of contacting you regarding your sessions.  You will be asked which method of contact you prefer and if it is okay to leave a message stating who we are.

​

Previous therapy details & mental/physical health diagnosis:- This helps us to assess your needs and plan therapy.

​

Doctors details: We need this information in case you are at risk of harm or others are at risk of harm.

Will we share your data and if we do, who will we share it with and for what purpose?

​

It is very unlikely that we will share your data.  We will not sell it on or use it for unethical reasons.

​

We may have to share if it our notes are subpoenaed by court, if you or anyone you tell us about, is at harm or risk of harm, we may have to pass this information on to your GP, Local Safeguarding Team or the police. We have appointed a Clinical Executor, so that in the unfortunately event our member of staff/sessional worker can no longer work with you; they will have access to your details and will get in touch with you.  You have a right to request to see, have amended or destroyed any data we may keep about you.  We will respond to any such request made within 28 days

How will we store your data?

The data on paper is kept in a locked filing cabinet, with personal data being stored separately from any session notes. Your phone number may be kept on my mobile phone, which is password protected.

How long will we store your data and how will we dispose of it?

​

We will keep your session notes, your personal details and your unique code for 7 years, which is the time frame we are legally obliged to keep them for.  This data will then be shredded.

​

We will delete your phone number from mobile devices 1 month after your sessions end.

​

All emails from you and about you will be deleted as soon as they are no longer needed and at least within 1 month of you ending with us.
